We are certified to facilitate:

• Diabetes Self-Management and Support 
• Chronic Disease Self-Management 
• Brief Tobacco Intervention 

The Foundation’s leadership and staff 

• Are subject matter experts 
• Members have served in leadership roles with other non-profits/NGOs 
• Members have previous service experience with local programs 
• Have strong community network, affiliations and contacts
